What Will I Need to Qualify?

For an individual sole proprietor to qualify for United Healthcare Group Insurance, you will need the following:

  • Current Business License
  • If in business for less than 1 year, you will only need your Business License
  • If in business for more than 1 year: IRS Schedule C (Form 1040)
  • If your business is set up as a corporation or other entity, Carl will walk you through the requirements.
